Patient Forms

Upon entering our office, our front desk staff will be ready to address your primary concerns.  We will ask that you spend a few minutes completing our patient forms in our office. This paperwork provides us with your health history and information on your condition.  We also ask that if you have health insurance to bring your insurance card so we may check your benefits and your coverage in our office.  This should take 10 minutes or less.

Chiropractic Consultation

Next, you will have a consultation with one of our doctors to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Glenn and Dr. Aryn to learn more about you, what brings you into our office, and your expectations in order to determine how chiropractic care can meet your goals.  Please be sure to bring any relevant health information or records of previous care you feel are appropriate for your condition or concerns, including recent x-rays if taken in the last year.  This should take 10 minutes or less.

Examination

After your consultation, a complete chiropractic examination testing your reflexes and flexibility will be performed. Standardized neurological, orthopedic, postural, and physical tests will be performed as well. However, nothing will be done in our office without your consent. If you have concerns regarding the severity of your case and your ability to perform these tests, we will address your concerns at the time of the first evaluation.  This normally takes 15-20 minutes, but is dependent of the level of attention needed according to the severity of your initial complaints.

X-Ray Studies

Depending on your specific condition, we may take X-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In most instances where X-rays are taken, we will be able to identify primary misalignments and degeneration, if present.

Treatment

At the conclusion of this initial evaluation, you will experience your first adjustment in our office.  Our care is highly interactive, so you can express concerns about the different styles of treatment at anytime.  The total time from forms to the completion of the first adjustment is normally 60 minutes.
